As a Home Health Registered Nurse, you'll provide individualized care in your patients' homes, helping them recover from illness, manage chronic conditions, prevent hospitalizations, and maintain their independence.

What You'll Do

• Perform comprehensive patient assessments
• Develop and implement individualized plans of care
• Provide skilled nursing services, medication management, wound care, and patient education
• Monitor patient progress and communicate changes to physicians and the interdisciplinary team
• Coordinate care with therapists, social workers, and home health aides
• Complete timely and accurate clinical documentation
• Support patients and families throughout their healthcare journey

Qualifications

• Active Virginia Registered Nurse (RN) License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment and Documentation Skills
• Excellent Communication and Organizational Skills
• Home Health Experience Preferred but Not Required
• Experience with OASIS, wound care, case management, or chronic disease management is a plus
