Home Health Registered Nurse (RN)Position Summary

We are seeking a qualified and experienced Registered Nurse (RN) to join our home health agency. The RN will be responsible for providing skilled nursing services to patients in their homes while also completing required clinical assessments, documentation, and case management duties.

The ideal candidate should have strong knowledge of home health regulations and documentation requirements, be comfortable working independently, and have experience with Kinnser/WellSky and OASIS.

Key Responsibilities

• Open and initiate new home health cases and coordinate the start of care.
• Perform comprehensive nursing assessments and OASIS assessments in accordance with Medicare and home health requirements.
• Develop and update individualized Plans of Care (POC) based on the patient's clinical needs.
• Provide skilled nursing services in the patient's home, including assessment, medication management, wound care, patient education, and disease management as applicable.
• Complete and maintain accurate and timely clinical documentation.
• Enter patient information, visits, assessments, orders, and other required documentation in Kinnser/WellSky.
• Review patient records and ensure all required documentation is completed and up to date.
• Coordinate care with physicians, therapists, HHAs, patients, family members, and other members of the healthcare team.
• Communicate with physicians regarding changes in the patient's condition and obtain necessary orders.
• Monitor patient progress and update the Plan of Care as needed.
• Complete required recertifications, resumptions of care, discharges, and other applicable home health documentation.
• Ensure documentation meets agency, Medicare, and regulatory requirements.
• Follow up on missing or incomplete documentation and ensure records are submitted within required timeframes.
• Participate in case management and assist with coordinating appropriate services for patients.
• Maintain patient confidentiality and comply with HIPAA and all applicable healthcare regulations.

Qualifications

• Current and unrestricted Registered Nurse (RN) license.
• Previous home health nursing experience required.
• Strong experience with OASIS assessments and Medicare home health documentation.
• Experience using Kinnser/WellSky or a similar home health EMR.
• Knowledge of Plan of Care (POC), physician orders, case management, and Start of Care (SOC) procedures.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage multiple patient cases.
• Excellent communication and organizational skills.
• Reliable, detail-oriented, and able to meet documentation deadlines.
• Current CPR certification and other required credentials.

Preferred Experience

• Experience opening and managing home health cases from referral through discharge.
• Familiarity with Medicare guidelines and home health compliance requirements.
• Experience coordinating PT, OT, ST, HHA, and other home health services.
• Knowledge of Kinnser/WellSky workflows and electronic clinical documentation.
