Responsibilities

• Provide direct patient care, including administering medications, performing bedside assessments, and developing individualized care plans
• Collaborate with patients, families, and healthcare teams to ensure effective communication and coordination of care
• Monitor patient progress, identify areas for improvement, and adjust care plans as needed
• Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
• Document patient information accurately and timely in accordance with facility policies and regulations
• Demonstrate a commitment to excellence in patient care, teamwork, and customer service

Requirements

• Current licensure as a Registered Nurse (RN) in the state of Texas
• Basic Life Support (BLS) certification
• Prior experience in a long-term care or similar setting
