Registered Nurse/Licensed Practical Nurse

We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our skilled nursing facility team. The ideal candidate will possess a strong background in patient care, with experience in long-term care, acute care, and specialized nursing practices.

$28.23 - $41.00 per hour

Duties

• Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to inform individualized care plans.
• Administer medications and treatments accurately according to physician orders while adhering to safety protocols and HIPAA regulations.
• Monitor residents’ conditions closely, recognizing changes that require intervention or escalation to medical teams.
• Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate comprehensive care strategies.
• Provide education and support to residents and their families regarding health conditions, medication management, and post-discharge instructions.
• Assist with procedures such as IV infusions, catheterization, wound care, and other advanced nursing interventions as required.
• Ensure infection control practices are rigorously followed to maintain a safe environment for residents and staff.

Skills

• Proven experience working in skilled nursing facilities with familiarity in long-term care regulations and compliance standards.
• Strong clinical skills, including patient assessment, medication administration, IV infusion, and wound management.
• Experience with specialized care areas such as Alzheimer’s/dementia care, memory care, hospice services, and behavioral health needs.
• Ability to perform triage effectively in urgent situations while maintaining calmness under pressure.
• Excellent communication skills for caregiver collaboration and client interaction across diverse populations, including geriatric patients and individuals with disabilities.
