Registered Social Worker

The STAR Family Health Team provides comprehensive primary health care services across multiple clinics, supporting a diverse team of healthcare professionals dedicated to delivering high-quality patient care.

We are seeking a Social Worker (MSW) to join our interdisciplinary team for a temporary 6 - month term. The role will support patients and families by providing a wide range of mental health supports including assessment, brief counselling and psychotherapy, crisis support, education, advocacy, care coordination, and system/community navigation.

The successful candidate will bring a compassionate, trauma-informed, patient-centred approach and work collaboratively with physicians, allied health professionals, and community partners, in addition to the following required qualifications:

• Master of Social Work (MSW) from a recognized university.
• Current registration in good standing with OCSWSSW.
• Minimum 3 years of relevant clinical experience, preferably in primary care or community health.
• Experience in counselling, psychotherapy, psychosocial assessment, crisis intervention, and supporting individuals and families.
• Knowledge of evidence-informed therapeutic approaches and relevant mental health legislation.
• Strong knowledge of local health, mental health, social, and community resources.
• Excellent interpersonal, communication, clinical, and organizational skills.
• Ability to work independently and collaboratively within an interdisciplinary team.
• Proficiency with EMRs and Microsoft Office.
• Valid driver's licence and reliable transportation required to travel across clinics.

Pay: $39.80 per hour

Expected hours: No more than 37.5 per week
