Remote Case Coordinator

The Case Manager / Care Coordinator provides support and guidance to individuals and families by helping them access healthcare, social services, and community resources. This role involves assessing client needs, coordinating services, monitoring progress, and maintaining regular communication with clients, families, caregivers, and service providers. The Case Manager helps ensure that clients receive appropriate services and support while promoting independence, safety, and overall well-being.

Responsibilities

• Assess client needs and help develop care or service plans.
• Schedule appointments, referrals, and support services.
• Communicate with clients, families, caregivers, and providers.
• Monitor client progress and follow up on services.
• Maintain accurate case notes, records, and documentation.
• Advocate for clients and connect them with community resources.
• Help identify barriers and find solutions to meet client needs.
• Protect client confidentiality and follow organizational policies.

Qualifications

• High school diploma or equivalent required; college degree preferred.
• Strong communication and interpersonal skills.
• Good organizational and time-management skills.
• Ability to handle multiple clients and responsibilities.
• Basic computer and documentation skills.
• Compassionate, dependable, and professional.
• Ability to work independently and as part of a team.

Pay: From $30.23 per hour

Expected hours: No less than 10.0 per week
