What they need

Are you an experienced MDS nurse who enjoys solving problems, supporting teams, and making an impact across different skilled nursing environments?

Health Dimensions Group (HDG)—a nationally respected leader in senior care consulting and management—is seeking an experienced MDS Consultant – RN to support skilled nursing communities throughout Minnesota.

In this role, you’ll step into communities where your MDS expertise is needed most—providing remote support, strengthening reimbursement and clinical documentation processes, ensuring accurate and timely assessments, and helping teams navigate the complexities of MDS and the Resident Assessment Instrument (RAI) process.

If you’re an experienced MDS professional who can quickly assess processes, identify opportunities, educate teams, and confidently provide hands-on or remote support when needed, this could be a great opportunity to put your expertise to work in a consulting environment.

Why this role stands out:

  • Use your MDS expertise across a variety of skilled nursing communities and operational environments
  • Make an immediate impact by supporting communities during vacancies, transitions, or periods of increased need
  • Partner with nursing, therapy, reimbursement, operations, and interdisciplinary teams
  • Help communities strengthen MDS accuracy, documentation, compliance, and reimbursement
  • Expand your experience through exposure to different communities, teams, systems, and operational challenges
  • Enjoy the variety of a travel-based consulting role while being supported by an experienced senior living consulting and management organization
  • Join a firm known for integrity, hospitality, and results-driven consulting

What you’ll do:

  • Provide on-site MDS consulting and hands-on support to skilled nursing communities throughout Minnesota
  • Complete and/or oversee accurate and timely MDS assessments in accordance with applicable federal and state requirements
  • Support communities with MDS coverage during vacancies, transitions, leaves of absence, or other periods of operational need
  • Review MDS processes, clinical documentation, and supporting records for accuracy, completeness, and compliance
  • Support appropriate reimbursement through accurate resident assessment, documentation, and coding practices
  • Provide guidance related to the RAI process, PDPM, care planning, and reimbursement
  • Collaborate with nursing, therapy, social services, dietary, and other members of the interdisciplinary team to ensure accurate resident assessments
  • Identify documentation or process gaps and provide practical recommendations for improvement
  • Educate and mentor MDS Coordinators, nursing leaders, and interdisciplinary team members on MDS requirements and best practices
  • Assist communities in developing effective systems and processes for assessment scheduling, completion, review, and submission
  • Support audit preparation, survey readiness, and regulatory compliance related to MDS and resident assessment requirements
  • Monitor key MDS and reimbursement indicators and identify opportunities for improved accuracy and performance
  • Communicate findings, risks, and recommendations clearly to community and organizational leadership
  • Help establish sustainable processes that community teams can maintain following the consulting engagement

What you bring:

  • Current Minnesota Registered Nurse (RN) license in good standing
  • Formal MDS experience in a skilled nursing/long-term care setting required
  • Strong working knowledge of the Minimum Data Set (MDS), Resident Assessment Instrument (RAI), and skilled nursing reimbursement processes
  • Knowledge and practical experience with PDPM
  • Experience completing, reviewing, and managing MDS assessments and associated documentation
  • Strong understanding of skilled nursing clinical documentation and interdisciplinary care planning
  • Knowledge of CMS requirements and regulatory standards applicable to skilled nursing facilities
  • Ability to review documentation, identify discrepancies or opportunities, and provide clear recommendations
  • Strong analytical, organizational, and problem-solving skills
  • Ability to educate, coach, and build relationships with nursing and interdisciplinary teams
  • Ability to work independently and quickly adapt to different communities, systems, and team environments
  • Flexibility and willingness to travel throughout Minnesota based on client and community needs
  • Previous MDS consulting, regional MDS, multi-site, or interim experience is strongly preferred
  • RAC-CT or other relevant MDS certification is preferred but not required

At HDG, you’re not just filling a staffing gap—you’re bringing specialized expertise to communities that need it. Backed by a company with more than 20 years of trusted leadership in senior care, you’ll have access to an experienced team of clinical, operational, financial, and workforce professionals who understand the complexities of skilled nursing.

If you’re an experienced MDS nurse looking for a dynamic, travel-based opportunity where you can use your expertise to strengthen teams, improve processes, and make an immediate impact, we’d love to connect.
