Remote Per Diem Licensed Nurse Practitioner/RN (CCM/RPM)

Full Job Description

We are seeking an experienced and highly organized Remote Per Diem Licensed Nurse Practitioner/RN with hands-on experience in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and/or Principal Care Management (PCM).

This role is focused on ongoing patient management and care coordination — not patient onboarding or enrollment only. The ideal candidate will be responsible for managing active patient panels, reviewing clinical data, coordinating care plans, and supporting positive patient outcomes through telehealth services.

Required Licenses

• Active Compact Nursing License (Mandatory)
• Additionally New York (NY) state license would be great to have as a plus

Required Experience

• Minimum 1–2 years of direct CCM/RPM/PCM experience
• Experience managing active patient panels and chronic care programs
• Telehealth and EHR documentation experience
• Strong patient follow-up and care coordination skills
• Experience with ongoing patient management (not intake/onboarding only)

Key Responsibilities

• Manage active CCM/RPM/PCM patient panels
• Conduct telehealth consultations and follow-up assessments
• Monitor RPM data and identify clinical concerns promptly
• Develop and update individualized care plans
• Coordinate care with physicians, caregivers, and patients
• Maintain accurate CMS/HIPAA-compliant documentation
• Support patient engagement, adherence, and quality outcomes
• Handle monthly patient rosters with accountability and professionalism

Compensation & Benefits

• $28–$30/hour based on experience
• Independent Contractor (1099)
