STARs Pharmacist | Remote Contract

We are actively hiring Clinical STARs Pharmacists located in Texas for an upcoming remote project focused on improving Medicare quality outcomes and closing care gaps. This is a great opportunity for pharmacists who enjoy patient interaction, medication therapy management, and working in a structured, metrics-driven environment.

Position Details

• Title: STARs Pharmacist
• Location: 100% Remote, must currently reside in Texas
• Start Dates: October 12, October 19, or October 26, 2026
• Schedule: Monday–Friday, 9:00 AM–5:30 PM

Position Overview

As a STARs Pharmacist, you will conduct telephonic medication reviews and outreach to Medicare members to improve medication adherence and close quality care gaps. You will provide comprehensive medication therapy management, screen for drug interactions, educate patients on chronic disease management, and collaborate with providers as needed.

This role operates in a high-volume, productivity-focused environment, with pharmacists expected to complete approximately 17–20 medication reviews per day while maintaining strong documentation accuracy and quality standards.

Key Responsibilities

• Conduct telephonic medication consultations with health plan members
• Identify and address medication adherence barriers
• Provide patient education on chronic conditions and medication use
• Apply clinical guidelines and coverage criteria
• Maintain HIPAA compliance and accurate documentation
• Meet weekly productivity and quality metrics

Required Qualifications

• Must currently reside in Texas
• Active, unrestricted Texas Pharmacist license
• PharmD from an ACPE-accredited program and successful NAPLEX completion
• At least 1 year of clinical, retail, or managed care pharmacy experience
• Ability to navigate multiple systems while engaging with patients
• Reliable high-speed wired internet and HIPAA-compliant workspace

Preferred Qualifications

• Call center or telephonic clinical experience
• Medication Therapy Management (MTM) experience
• Experience with Medicare STARs and HEDIS measures
• Bilingual candidates encouraged to apply
