Junction Support Services was established with one goal in mind, to support vulnerable people in our community.

Every day, our people work alongside children, young people, individuals and families, empowering brighter futures and building strong, thriving communities together.

About the Role

As a Residential Care Case Manager at Junction Support Services, you will provide trauma-informed, therapeutic and person-centred case management to children and young people living in residential care. Working collaboratively with young people, families, carers and key stakeholders, you will support the development and implementation of individualised case plans that promote safety, stability, wellbeing and positive life outcomes.

You will advocate for young people, coordinate services, facilitate meaningful participation in decision-making, and help strengthen connections to family, culture, education and community. Through building trusted relationships and identifying strengths, you will empower young people to achieve their goals and develop the skills needed for independence and future success.

This role is about much more than case coordination. It is about creating opportunities, fostering resilience and walking alongside young people as they navigate challenges and work towards brighter futures.

This is a full-time opportunity based in Morwell, working within a dedicated and supportive Residential Care team committed to making a lasting difference in the lives of vulnerable children and young people.

What You Can Expect

In this role, you will:

  • Build strong relationships with young people, families, carers and key stakeholders to achieve meaningful and sustainable outcomes.
  • Advocate for young people and coordinate access to education, health, therapeutic, cultural and community-based supports.
  • Strengthen connections to family, culture, identity and community, supporting a sense of belonging and resilience.
  • Lead care team collaboration, maintain quality documentation and support successful transitions toward independence and future goals.

This role is about more than case management. It is about creating opportunities, building resilience and supporting children and young people to thrive in a safe, stable and nurturing environment.

About You

You will bring:

  • Relevant qualifications in Community Services, Social Work, Welfare, Psychology or a related discipline, and/or extensive experience in case management within child, youth and family services.
  • Demonstrated experience working with children and young people who have experienced trauma, abuse, neglect and other complex needs.
  • Strong knowledge of trauma-informed, therapeutic and strengths-based practice, with the ability to engage and support vulnerable young people.
  • Well-developed case management skills, including assessment, care planning, advocacy, stakeholder engagement and outcome-focused interventions.
  • Experience collaborating with families, carers, government departments and community service providers to achieve positive outcomes for young people.
  • Excellent communication, relationship-building and organisational skills, with an understanding of Child Safe Standards, permanency planning and relevant legislative frameworks.
