Responsibilities Include

• Providing daily living support and coaching independent living skills.
• Implementing individualized care and behavior support plans.
• Monitoring health, safety, and assisting with personal care as needed.
• Facilitating transportation and promoting community integration.
• Documenting progress and communicating with supervisors and clinical staff.
• Creating a structured, compassionate, person-centered environment to help residents achieve their goals.
