Full-time 35 hours per week

$5,000.00 sign on bonus

Community Nurse is looking for a full-time Admissions Registered Nurse to join the Home Care Nursing Team. This position has weekend and holiday rotations.

Requirements:

• Licensed RN in Massachusetts.
• Graduate of an approved school for professional nursing.
• A minimum of one year of recent experience in direct patient care nursing.
• Previous home care experience preferred.
• Experience with documentation in a patient Electronic Medical Record is a plus.

Responsibilities include but not limited to:

• Give skilled comprehensive nursing care to all age, gender, and cultural groups.
• Assess client status upon initial home visit to identify needs in support of development of comprehensive case management plan.
• Document in an accurate and timely manner not later than end of working day.
• Using basic teaching principles, counsel and instruct the patient, family and/or significant other in meeting nursing and related needs.
• Maintain regular communication with the Clinical Manager, and/or other team members concerning patient/family needs.
• Participate in Community Nurse’s orientation and in-service training programs for professional staff.
• Participate in agency and community programs as requested to promote professional growth and understanding of home care.
• Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies.
• Accept other assignments as appropriate.

Job Type: Full-time

Pay: $38.00 - $43.00 per hour
