RN Case Manager

Summary

Hillside Manor Rehabilitation and Extended Care Center, LLC, a 400 bed Skilled Nursing Facility seeking an experienced, dynamic, and knowledgeable RN to be responsible to perform Case Management functions for our short-term rehabilitation population.

Responsibilities

• Performing utilization reviews for short term managed care members
• Will serve as a strong and knowledgeable liaison between social workers, rehab therapists, nursing and physicians-all in support of patients and their families-and coordinate care
• Providing education and support to residents and their families on managing health conditions, accessing resources, and making informed decisions
• Negotiating level of payment based on a contractual agreement with managed care companies
• Handling managed care appeals and denials in a timely manner
• Assisting with discharge planning
• Communicate effectively with accounting department for appropriate coding and billing
• Performing monthly communication for long-term MLTC members as per DOH regulations
• Ability to work collaboratively with a multidisciplinary team
• Experience using EMR like SigmaCare or PCC

Requirements

• Current Registered Nurse (RN) license in the state of New York.
• 2 year' experience in case management or a similar role preferred.

Compensation will be commensurate with experience, and a comprehensive benefits package is included.
