Job Overview:

We are seeking an Hospice RN Case Manager to oversee and manage a caseload of 10-15 patients. Work closely with Interdisciplinary Team and Medical Director to create a Plan of Care designed specifically to meet each patients and family Hospice needs.

Responsibilities:

• To see each patient as needed per Care Plan frequency. Manage a medicaton box for each patient. Order DME, supplies and medications as needed.

• On-call rotation, once every 4 weeks.

• Attend and participate in weekly IDT meeting.

• Must be self motivated.

• Time management.

Experience:

• Previous experience in Geriatric Care and care settings, i.e Personal Care Homes, Nursing Homes, Assisted Living Facility.

Job Type: Full-time

Pay: $72,538.00 - $79,305.00 per year

• Case management: 1 year (Preferred)
