JRA Health is a new and emerging home infusion provider, looking for talented individuals to assist in the organization’s focus on filling the gap in acute care home infusion services. JRA Health is licensed & accredited to provide infusion services to patients in the Florida market and beyond.

Job Description Summary:

This is an on-site, full-time role located in Jacksonville, FL, for a Registered Nurse (RN) Case Manager. The RN Case Manager will manage patient care by coordinating discharge planning, performing utilization management, supporting patients and their families, and ensuring seamless continuity of care. The role requires collaboration with interdisciplinary teams to assess medical needs, provide patient education, and develop personalized care plans aimed at optimizing health outcomes.

Job Responsibilities:

• Work with internal (intake & operations personnel) and external (case manager, referring provider, etc.) to safely and efficiently onboard and transition patient infusion care to JRA Health.
• Collaborate with authorizations team in the clinical review and submission process for prior authorizations.
• Assist billing team with troubleshooting medical ICD 10 code claim denials
• Participate in ongoing quality assurance program by completing clinical assessments post-admission to evaluate success of admission, start of care visit, and address any clinical concerns in coordination with internal (clinical, sales & ops team) and external stakeholders (home health agencies)
• Assist field team in providing bedside education & IV hookups as needed.

Required Qualifications:

• Minimum 3 years of inpatient clinical case management experience
• Registered Nurse Licensure in State of Florida
• Proficiency in Microsoft 365 environment (Excel, Word, Teams, Outlook, PowerPoint, etc.)
• Good time management skills
• Passion and empathetic approach to helping patients & referral sources
• Effective problem solving & communication skills
• Effective Team Player

Physical Requirements:

• Ability to communicate orally with customers, management, and other co-workers, both individually and in front of a group is crucial. Regular use of the phone and e-mail for communication is essential.
• Sitting, driving, walking, standing for extending periods. Hearing and vision within normal ranges is essential for normal conversations, to receive ordinary information and to prepare or inspect documents.
• No heavy lifting expected; occasional exertion of up to 25 lbs. of force may be required. Good manual dexterity for the use of common office equipment.
