RN Case Manager – Full Time

Elite Home Care is seeking a compassionate, organized RN Case Manager to coordinate care for patients receiving skilled home health services. You’ll work with patients, families, physicians, and our clinical team to keep care plans clear, timely, and focused on each patient’s goals.

What you’ll do

• Complete skilled nursing visits and patient assessments in the home.
• Develop and update individualized plans of care in collaboration with physicians and the care team.
• Manage a patient caseload, monitor progress, and identify changes that need follow-up.
• Provide medication education, disease management teaching, and support for patients and caregivers.
• Coordinate services with therapy, home health aides, physicians, and other providers.
• Complete OASIS assessments and clinical documentation accurately and on time. Elite’s expectation is that visit notes and OASIS documentation are completed within 24 hours.

What we’re looking for

• Current, unrestricted Oklahoma RN license.
• Strong assessment, communication, and time management skills.
• Ability to work independently while staying connected with the clinical team.
• Reliable transportation and the ability to travel to patients’ homes in the Oklahoma City metro area.
• Home health and OASIS experience preferred.
