RN Case Manager-HCHB Experience

Patient Assessment: Conduct physical evaluations, monitor vital signs, and track recovery progress during each visit.

• Clinical Care: Administer prescribed medications, manage IV therapies, provide wound care, and oversee medical equipment like feeding tubes or oxygen.
• Care Coordination: Partner with doctors, physical therapists, and family members to update and execute personalized care plans.
• Patient & Family Education: Teach caregivers how to manage treatments, recognize warning signs, and improve home safety.
• Documentation: Record accurate clinical notes and symptoms using electronic medical record (EMR) systems.
• Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program.
• Licensure: Active, unrestricted RN license in the state of practice.
• Experience: Typically requires 1+ years of general nursing or clinical care experience; previous home health experience is often preferred.
• Travel Essentials: Valid driver’s license, reliable personal vehicle, and auto insurance, as travel to multiple homes is a daily requirement.
• Physical & Soft Skills: Ability to lift up to 50 pounds, strong independent problem-solving skills, and deep empathy.

Job Type: Full-time

Pay: $85,000.00 - $90,000.00 per year
