RN Case Manager – Home Care

Base salary + Company Car | Washington, DC & Montgomery County, MD

Job description:

We are seeking a dedicated and compassionate Nurse Case Manager (RN) to join our growing home care team. This full-time Nurse Case Manager (RN) position focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in the Washington, DC and Montgomery County, MD communities. Schedule: Can be flexible with Days and Hours
Monday through Friday
Work Schedule: 9:00 AM – 5:00 PM

RN Case Manager Key Responsibilities:

• Conduct initial client assessments and routine reassessments in home settings
• Conduct hospital discharge assessments
• Develop and update individualized care plans
• Collaborate with caregivers, families, and interdisciplinary teams
• Monitor client progress and ensure compliance with care standards
• Maintain accurate and timely documentation
• Completing annual skills assessments and competencies for LPNs and CNAs/Caregivers,
• Training and supervising healthcare personnel: LPNs, CNAs, and Caregivers.
• Attend dischange planning as needed

Qualifications:

• Active Maryland and DC RN license in good standing (Compact/Multi-state)
• At least two years of clinical experience (home care, case management, or patient assessment)
• Strong clinical judgment and communication skills
• Ability to work independently and manage a flexible schedule
• Current CPR Certification, AHA BLS

Pay: $85,000.00 - $100,000.00 per year
