StartDate: 12/7/2026 Pay Rate: $1800.00 - $2500.00

POSITION SUMMARY – RN Case Manager/Care Coordination POSITION DUTIES – Coordinate patient care across providers, oversee utilization review to ensure appropriate and cost-effective services, and facilitate timely, safe discharge planning by arranging post-acute resources and support.

Minimum Required Qualifications –

• CA RN license
• BLS, ACLS (AHA)
• 2 years hospital experience and acute level care coordination experience
• Skilled experience with discharge planning and utilization review
• Prior CA Dignity/Commonspirit acute care experience in specialty

PREFERRED QUALIFICATIONS – Minimal Time Off Requests LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8hr Days, EOW

SYSTEMS – Cerner
