RN - Clinic based

Registered Nurse (Clinic-based) Full-time

7:30 a.m. ? 4:00 p.m.

$75k - $82k

Responsibilities Include:

• Conducts assessments for new enrollees and periodic reassessments for current participants.

• Coordinate with Home Care Coordinator to ensure CSC participants receive appropriate home care.

• Contributes to the development and revision of participant care plans.

• Completes Nursing documentation and ensures completion, accuracy and relationship to priorities and goals.

• Participates in providing 24/7 ?on call? coverage in collaboration with other professional staff, as assigned.

• Attend CSC team meetings, care plan meetings, and including scheduled meetings with participants and/or families.

Education, Training And Experience:

• Registered Nurse and be a graduate of a school of professional nursing.

• Certified in CPR

• Three years? experience in nursing

• Minimum one year nursing experience with home care, geriatrics, case management, or discharge planning with frail elderly population in acute long-term care, or community-based setting.

• In good standing with the Medicare and Medicaid programs
