At Elara Caring, we’re looking for an RN Clinical Team Manager who enjoys coaching others, solving problems, and keeping patient care moving forward. You’ll guide a team of home health clinicians, oversee care coordination, and partner with branch leadership to support quality care and a collaborative team environment.

  • Continued connection to patient care through field support as needed

How You’ll Make an Impact

  • Lead daily clinical operations: Coordinate patient and staff assignments, referrals, and timely starts of care.
  • Guide individualized care: Oversee the development, implementation, and ongoing updates of patient plans of care.
  • Put your OASIS expertise to work: Conduct regular OASIS reviews and evaluate care plans and proposed changes for clinical appropriateness.
  • Keep care connected: Coordinate physician orders and communication among clinicians, patients, families, providers, vendors, and facilities.
  • Coach and develop your team: Meet regularly with clinicians to provide guidance, address concerns, and communicate organizational updates.
  • Help build a strong team: Participate in interviews and recommend candidates for direct care positions.
  • Partner with leadership: Communicate with the Branch Director or Administrator about clinical needs, staffing, and key performance metrics.
  • Provide hands-on support: Remain available for fieldwork within the branch coverage area as needed.
  • Complete additional duties and projects as assigned.

What You Bring

  • Current, unrestricted RN license valid in the state of practice
  • Skilled home health and OASIS experience required
  • At least 2 years of RN experience in home health or hospice, including the required skilled home health experience
  • Associate degree in nursing or a related nursing field
  • 1 year of supervisory experience preferred
  • Ability to become proficient in Homecare Homebase and Workday, along with knowledge of Microsoft Office
  • Ability and willingness to travel throughout the branch coverage area
  • A valid driver’s license and acceptable driving record, or reliable transportation between patient homes and assigned work locations
  • Ability to work in a physically demanding, high-stress environment and frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning, transfers, and equipment
