RN Enhanced Care Management

We are expanding our Enhanced Care Management (ECM) program and are seeking a dedicated Registered Nurse (RN) to join our multidisciplinary Care Team. This role is ideal for an RN who is passionate about working with vulnerable populations, providing whole-person care, coordinating services, and meeting clients where they are—in their homes, in the community, or alongside our mobile mental-health unit.

As an essential member of the ECM Care Team, the RN plays a central role in improving client health outcomes, reducing barriers, and supporting high-need individuals with complex medical, behavioral, and social challenges.

Key Responsibilities

Care Management & Clinical Support

• Conduct comprehensive health assessments, including medical, behavioral, and social needs.
• Develop and update person-centered care plans in collaboration with the ECM team.
• Provide client education on chronic disease management, medications, and preventative care.
• Deliver brief clinical interventions that support improved self-management.

Care Coordination

• Collaborate closely with BH Care Managers, Care Coordinators, and CHWs.
• Facilitate warm hand-offs with medical providers, behavioral health, specialty care, and social service agencies.
• Ensure closed-loop referrals and follow-up on care plan milestones.

Community & Field-Based Support

• Provide services in the field, including client homes, hospitals, shelters, and community locations.
• Participate in mobile outreach with Move With Hope™ to reach underserved clients directly.
• Support crisis intervention efforts and link clients to immediate resources.

Documentation & Compliance

• Complete timely documentation in accordance with DHCS ECM guidelines.
• Participate in weekly case reviews, multidisciplinary rounds, and program training.
• Maintain compliance with HIPAA, state nursing regulations, and organizational standards.

Qualifications

• Current California RN License in good standing.
• Experience with case management, public health nursing, behavioral health, or community-based work preferred.
• Comfort with field-based work and engaging vulnerable, high-needs clients.
• Ability to work collaboratively within a multidisciplinary team.
• Strong communication, empathy, and problem-solving skills.
• Must have valid CA driver’s license and reliable transportation.
