We are looking for a Registered Nurse to join our team, full time. The ideal candidate will be compassionate, have excellent communication skills, and be able to work independently. This is an excellent opportunity for a Registered Nurse to join a dynamic team and make a positive impact on the lives of our patients.

Responsibilities:

• Facilitate Care coordination
• Provide direct patient care.
• Administer medications and treatments as prescribed by the physician.
• Participate in quality improvement activities.
• Maintain accurate records of patient care activities.
• Supervise floor staff.
• Perform Assessments prior to admit, upon return and as needed.
• Manage medication orders and oversee service delivery of medication administration.
• Create service plans for Residents based on needs
• Work with waiver program case managers
• Communicate with providers and manage emergency situations.
• Other duties as needed.

Job Type: Full-time

Pay: $38.00 - $42.00 per hour
