RN Home Health Case Manager

Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their health and well-being while promoting independence. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care.

Responsibilities

• Conduct comprehensive assessments of patients' health status, including vital signs and medical history.
• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
• Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.
• Provide education to patients and families regarding disease management, medication administration, and health maintenance.
• Assist with discharge planning to ensure a smooth transition from hospital to home care.
• Maintain accurate documentation of patient care activities in compliance with HIPAA regulations.
• Collaborate with interdisciplinary teams to coordinate care for patients with complex needs, including those requiring infectious disease management or pediatric care.
• Respond promptly to patient inquiries and concerns, delivering exceptional patient service at all times.

Skills

• Strong clinical skills in nursing practice, particularly in home health settings.
• Proficient in assessing vital signs and recognizing changes in patient conditions.
• Knowledge of discharge planning processes and effective communication strategies with patients and families.
• Familiarity with HIPAA regulations to ensure patient confidentiality and data protection.
• Experience in pediatrics or infectious disease care is a plus but not required.
• Excellent interpersonal skills with the ability to build rapport with patients from diverse backgrounds.
• Ability to work independently while managing multiple priorities effectively.
