Position Summary
We are seeking a PRN RN Field Nurse to provide skilled home health visits to patients in and around Michigan City and La Porte, Indiana. This role offers flexible, as-needed scheduling and is ideal for a nurse who values autonomy, meaningful patient relationships, and the ability to make a direct impact in the community.

Key Responsibilities

• Conduct in-home nursing visits per the physician-directed plan of care
• Perform skilled nursing tasks including wound care, medication management, injections, and vital sign monitoring
• Assess patient condition and communicate changes to the care team and physician
• Educate patients and family members/caregivers on disease management, medications, and home safety
• Maintain accurate, timely documentation in compliance with agency and regulatory standards
• Coordinate care with interdisciplinary team members as needed

Qualifications

• Active, unencumbered RN license in the state of Indiana
• Current CPR certification
• Reliable transportation and willingness to travel to patient homes in the Michigan City and La Porte, Indiana area
• Strong clinical judgment, communication, and time-management skills
• Prior home health experience preferred but not required

Pay: $45.00 - $85.00 per hour

Expected hours: 45.0 – 85.0 per week

• Nursing: 1 year (Required)
• Home health: 1 year (Preferred)

License/Certification:

• RN License (Required)
