Registered Nurse – Minimum Data Set (MDS RN) Job Summary The MDS Registered Nurse (MDS RN) is responsible for coordinating and completing the Minimum Data Set (MDS) assessment process to ensure accurate resident assessments, regulatory compliance, and appropriate reimbursement. Collaborates with the interdisciplinary team to develop individualized care plans, maintain documentation, and support quality resident care in accordance with federal, state, and facility guidelines.

Requirements

• Associate Degree in Nursing (ASN) required; BSN preferred.
• Current Registered Nurse (RN) license in the state of practice.
• Current BLS certification preferred or required per facility policy.
• Recent MDS, long-term care (LTC), or skilled nursing facility (SNF) experience preferred.
• Working knowledge of RAI/MDS guidelines and Medicare/Medicaid regulations preferred.

Responsibilities

• Complete and coordinate MDS assessments in accordance with federal and state regulations.
• Ensure accurate and timely MDS documentation and submission.
• Develop and update resident care plans with the interdisciplinary team.
• Review clinical documentation to support coding accuracy and reimbursement.
• Monitor resident assessments and communicate changes in condition.
• Maintain compliance with CMS, Medicare, Medicaid, and facility policies.
• Educate and collaborate with nursing staff regarding MDS and care planning requirements.
• Maintain accurate clinical documentation and resident records.
• Perform other duties as assigned.
