REPORTS TO: Clinic Manager

SUMMARY/OBJECTIVE: Provides professional team-based care to patients. Assists in the continuum of care and in educating patients on their patient centered medical home.

ESSENTIAL JOB DUTIES: (This list may not include all of the duties assigned)

• Performs general nursing care to patients. Screens patients for appropriate information. Administers prescribed medications, tests and treatments in accordance with nursing standards.
• Utilize nursing knowledge and skills in the safe implementation of basic preventative and therapeutic nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
• Assists with the preparation of equipment and aids physician during treatment, examination, and testing of patients and documents accurately in the patient's chart.
• Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards.
• Responds to and refers incoming telephone calls. Instructs patient and family regarding medications and treatment instructions, within scope of practice.
• Responsible for preparation of exam rooms including re-stocking of treatment areas.
• Work collaboratively with other members of the health care team in coordination of the patient's care as evidenced by other department feedback.
• Participate in professional development activities and maintains professional affiliations.
• Performs related work as required.

EDUCATION AND EXPERIENCE:

RN

Required:

• Graduation from an accredited school of nursing
• Possession of a State Registered Nurse License
• CPR Certification

Preferred:

• One year of professional nursing experience in a clinic setting preferred
• Word processing and computer experience

LPN

Required:

• Graduation from an accredited school of practical nursing.
• Valid state license to practice practical nursing
• CPR Certification

Preferred: Word Processing and computer experience.
