SKILLS REQUIRED

Primary (Must-Have):

1. • Registered Nurse (RN)
2. • BLS certification
3. • Case Management experience
4. • Ability to work a full-time 40-hour weekly schedule
5. • Active California RN license

Secondary (Good to Have):

1. • Healthcare utilization management experience
2. • Experience working in a hospital or clinical healthcare setting
3. • Strong communication and coordination skills
4. • Experience with care coordination and patient management

POSITION OVERVIEW

The Case Manager will provide professional case management services within the Nursing Staffing Office at Downey Medical Center. The role involves coordinating patient care, supporting appropriate utilization of healthcare services, and collaborating with healthcare professionals to promote quality patient outcomes. The position requires an experienced RN with strong clinical judgment, communication, and care coordination skills.

ROLES & RESPONSIBILITIES

1. • Provide case management and care coordination services for patients.

• Coordinate with physicians, nurses, and other healthcare professionals to support appropriate patient care.

• Assess patient needs and assist with developing and coordinating individualized care plans.

• Support appropriate utilization of healthcare resources and services.

• Monitor patient progress and coordinate services across the continuum of care.

• Communicate effectively with patients, families, providers, and interdisciplinary teams.

• Maintain accurate and timely documentation related to patient care and case management activities.

SCHEDULE

Monday–Friday | 8:30 AM–5:00 PM | 8-hour shift | 40 hours/week
