RN/LPN

Overview

We are seeking a compassionate and dedicated RN/LPN to join our home care team. In this role, you will provide personalized nursing care to patients in their homes, ensuring they receive the highest quality of medical attention and support. The ideal candidate will be responsible for assessing patient needs, administering treatments, and collaborating with families and healthcare providers to promote health and well-being.

Responsibilities

• Deliver direct nursing care to patients in a home setting, including medication administration, wound care, and vital sign monitoring.
• Assess patients' health conditions regularly and update care plans accordingly.
• Educate patients and their families on managing health conditions, medications, and treatment procedures.
• Coordinate with physicians and other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and detailed documentation of all patient interactions, treatments, and observations.
• Ensure compliance with all safety protocols, healthcare regulations, and privacy standards.
• Provide emotional support and encouragement to patients during their recovery or ongoing care process.

Requirements

• Valid RN or LPN license in the state of practice.
• Prior experience in home health or related healthcare settings is preferred but not mandatory.
• Strong clinical skills with the ability to perform assessments and deliver appropriate interventions.
• Excellent communication skills for effective interaction with patients, families, and healthcare teams.
• Compassionate attitude with a commitment to providing personalized patient-centered care.
• Ability to work independently while managing multiple patient cases efficiently.
• Reliable transportation and flexibility to work various shifts as needed.
