Position Summary: RN/LVN Charge Nurse

As a Charge Nurse (RN/LVN), you will play a vital role in providing high-quality care and promoting the well-being of each resident based on their individual care plans and assessments. This position is ideal for experienced, passionate nurses who want to make a meaningful difference in a skilled nursing facility setting.

Key Responsibilities:

• Monitor, assess, and document resident conditions during rounds.
• Implement and follow care plans as directed.
• Administer prescribed medications accurately.
• Participate in physician rounds and chart visits.
• Perform narcotic reconciliation between shifts.
• Identify significant changes in resident health and take appropriate action.
• Oversee and support nursing aides during your shift.
• Complete incident and accident reports as per policy.
• Conduct resident discharges in accordance with procedures.
• Complete nursing assessments before contacting physicians.
• Communicate clearly with residents and their families regarding health updates.
• Perform other tasks as assigned by the Director of Nursing or Administrator.

Qualifications:

Education:

• Graduate of an accredited RN or LVN program.

Licensure:

• Active RN or LVN license in good standing in the state of Texas.

Experience:

• Previous experience in a skilled nursing facility or long-term care setting preferred.
