RPM Facility Relations Representative

Job Summary
We are seeking an energetic and proactive RPM Facility Relations Representative to serve as a vital link between our organization and the facilities we partner with. In this role, you will be responsible for fostering positive relationships, ensuring smooth communication, and coordinating operational activities across multiple facilities. Your enthusiasm and dedication will help streamline processes, resolve issues promptly, and promote a collaborative environment that supports our organizational goals.

Location: In-person | Full-time (Monday–Friday, 8 a.m.–5 p.m.)

This position requires driving to 4-5 facilities daily.

About the Role
We’re looking for a personable and reliable professional with excellent customer service skills to serve as the main point of contact between our practice and Assisted Living Facilities. No prior clinical experience is required—we provide full training on taking vital signs and managing onsite facility needs.

Responsibilities

• Visit Assisted Living Facilities to take patient vital signs (training provided)
• Communicate with Wellness Directors, Administrators, and staff regarding patient or facility concerns
• Direct facility or patient requests to the appropriate operational teams
• Serve as an ambassador for our practice and maintain positive relationships with facility staff and patients
• Assist with connecting patients and facilitating insurance transitions

Qualifications

• Excellent verbal communication and organizational skills
• Friendly, professional demeanor with strong social awareness
• Effective follow-up and coordination skills across departments
• High School Diploma required
• Reliable personal transportation required
