What they need

Job Overview

The Sales Liaison is Healthmonitor1's face in the field. You work a daily route of healthcare facilities, building and maintaining relationships with facility leadership, setting up provider meetings, gathering the documents needed to credential our providers, and making sure every facility has the consents and contact information we need to serve it well. You keep GoHighLevel accurate and current, and you report to the Operations Manager at the end of every day.

This role combines two jobs in one: opening new referral partners (prospecting, qualifying, presenting our care model, and onboarding the facility) and growing the partners we already have (ongoing education, referral monitoring, problem resolution, and finding new opportunities inside each account). Partner types include Skilled Nursing Facilities, Assisted Living Facilities, Home Health agencies, Hospital Case Management, Primary Care, Specialists, ACOs, and Health Systems.

Compensation of $25 per hour with commission potential

Key Responsibilities

Route and daily planning

  • Check the day's route and facility list before heading out, confirming each stop and whether it is a new or follow-up facility.
  • Prospect pipeline: build and maintain a list of target facilities and decision makers using web research, professional networking, and LinkedIn, and work new targets into the route.
  • Outreach between stops: use phone and email between visits to set introductory meetings, confirm appointments, and follow up on open items.

New facilities

  • Establish the relationship with facility leadership on the first visit.
  • Schedule follow-up immediately in GoHighLevel before leaving the facility.
  • Schedule a provider meeting with the Director of Nursing (DON) or other appropriate facility personnel.
  • Collect credentialing documents required to credential our providers at the facility**.**
  • Competitor check: verify whether a competitor is on site and, if so, record the competitor's name.
  • Qualify the facility: confirm current provider relationships, patient volume/census, service need, who makes the decision, and how ready the facility is to start
  • Present and educate: present Healthmonitor1's care model, screening tools, and referral process to facility leadership and clinical staff, and leave collateral behind.
  • Advance the opportunity: move each new facility through the pipeline stages (prospect, qualified, presentation, agreement, implementation, active partner)
  • Hand off for launch: coordinate the launch with the Clinical Team and Operations so the first referrals and admissions go smoothly.

Follow-up Facilities

  • Confirm all facility and contact information is current and correct anything that has changed.
  • Schedule the next DON and provider meeting.
  • Reassure the facility that it will receive a monthly report at the end of each month.
  • Referral growth: monitor each facility's referral activity, spot gaps or drops, and look for ways to increase referrals or introduce additional services.
  • Ongoing education: train new facility staff, refresh the referral process, provide marketing materials, and coordinate in-services or lunch & learns.
  • Voice of the facility: gather facility feedback, resolve concerns or escalate service/clinical issues to Operations, and report what you learn about the market and competitors.
  • Leadership check-ins: hold periodic check-ins with the Executive Director and corporate contacts, not only the DON.

Contact Management (all facilities)

  • Obtain and keep current a name, a good contact number, and an email address for the Director of Nursing, primary contact, Executive Director, corporate contact (if the facility has one), and Medical Director.

Consent Forms and Compliance

  • Confirm every facility has the necessary consent forms on file.
  • When any are missing, connect with the DON and provider to obtain them promptly.

Documentation and Reporting

  • Facility-by-facility updates: enter all visit information into GoHighLevel before proceeding to the next facility, leaving no gaps in documentation.
  • Pipeline stage: keep each facility's pipeline stage, notes, disposition, and next steps current in GoHighLevel.
  • Advance scheduling: leave every facility with the next follow-up visit already booked and recorded in GoHighLevel.
  • Data integrity: ensure GoHighLevel is up to date with all facility information, with no discrepancies, every day.
  • End-of-day report: email a report to the Operations Manager every day. This is mandatory.

Daily Expectations

  • Submit an end-of-day report by email to the Operations Manager — every day, without exception.
  • Update GoHighLevel completely for each facility before leaving it and moving to the next.
  • Leave every facility with the next follow-up visit already scheduled and entered in GoHighLevel.

Performance Measures

Success in this role will likely be measured by the following. Specific targets are set by the Operations Manager.

  • Activity: facility visits completed per day/week and provider/DON meetings scheduled.
  • New partners: new facilities opened, onboarded, and actively referring.
  • Referrals: referral volume and growth per facility, and partner retention.
  • Compliance: credentialing documents and consent forms collected and complete.
  • Reporting: GoHighLevel accuracy and on-time end-of-day reports.

Qualifications

Required

  • Experience in sales, business development, or healthcare liaison work, ideally with skilled nursing, long-term care, or other facility-based settings.
  • Comfort meeting with clinical and administrative leaders, including DONs, Executive Directors, and Medical Directors.
  • Strong organizational skills and the discipline to document each visit before moving on.
  • Clear written communication for daily reporting.
  • Reliable transportation and a valid driver's license for daily route travel.
  • Track record of meeting quotas, visit goals, or other KPIs.

Preferred

  • Working knowledge of GoHighLevel or a comparable CRM.
  • Familiarity with provider credentialing and consent documentation.
  • Established network of contacts in the Florida healthcare market (SNF, ALF, home health, hospital case management, primary care).
  • Clinical liaison or healthcare account management experience; experience generating provider referrals.
  • Comfort with Google Workspace and Slack.

Core Competencies

  • Relationship building: earns trust quickly and keeps it through consistent follow-through.
  • Attention to detail: contact data, documents, and CRM records are complete and accurate.
  • Time management: runs a multi-stop route on schedule without sacrificing documentation.
  • Accountability: reports honestly and on time, every day.
  • Presentation and education: comfortable leading facility presentations and in-services with clinical and executive audiences.
  • Consultative approach: listens for the facility's needs and positions our services as the solution.
  • Persistence: keeps going through no-shows, gatekeepers, and slow decisions.
  • Responsiveness: answers facility questions and resolves issues quickly.
  • Collaboration: works closely with Clinical, Operations, and leadership to deliver on what was promised.

Working Conditions

Field-based role with daily travel between facilities. Time is split between on-site visits and documentation. Facility visits are conducted in line with each facility's entry and visitor policies
