What they need
Position Summary
New Vision Health & Wellness is seeking a dependable, compassionate, and organized School-Based Case Manager to provide case management services to students receiving behavioral health services through our organization.
The School-Based Case Manager will work directly within Cleveland-area schools to support students and help address barriers that may affect their behavioral health, academic success, attendance, family stability, and overall well-being.
This position requires someone who is comfortable working independently in school environments while maintaining strong communication with students, families, school personnel, and the New Vision clinical and administrative teams.
Key Responsibilities
- Provide school-based case management services to students assigned to your caseload.
- Meet individually with students based on their identified needs and service plans.
- Assess student and family needs and identify barriers affecting the student's stability, functioning, or access to services.
- Assist students and families with accessing community resources and supportive services.
- Coordinate care and communicate with parents/guardians, school personnel, behavioral health providers, and other involved professionals as appropriate.
- Advocate for students and families when additional services or resources are needed.
- Support students with areas such as school attendance, behavioral concerns, social and emotional needs, family challenges, access to healthcare, transportation, food insecurity, housing instability, and other social determinants of health.
- Monitor student progress and follow up on referrals and identified needs.
- Participate in care coordination and case consultation with the New Vision team as needed.
- Maintain accurate, timely, and complete case management documentation.
- Complete required documentation in accordance with New Vision policies, payer requirements, and applicable state and federal regulations.
- Maintain student and family confidentiality and comply with HIPAA and other applicable privacy requirements.
- Maintain professional relationships and appropriate boundaries with students, families, school personnel, and community partners.
- Travel between assigned schools and other approved service locations as necessary.
Qualifications
- Previous case management experience required.
- Previous experience working with children, adolescents, or families required.
- Experience working in behavioral health, social services, community-based services, schools, or a related setting strongly preferred.
- Ability to build positive and professional relationships with children and adolescents.
- Strong communication, organization, time-management, and documentation skills.
- Ability to manage an assigned caseload and work independently in a school-based environment.
- Ability to collaborate effectively with families, school personnel, behavioral health professionals, and community organizations.
- Comfortable working with students and families from diverse backgrounds and circumstances.
- Basic computer skills and ability to complete electronic documentation.
Transportation Requirements
Because this is a community and school-based position, candidates must have:
- A valid driver's license.
- Reliable personal transportation.
- Current automobile insurance.
- Ability and willingness to travel to assigned schools throughout the Cleveland area.
Schedule
This is a caseload-based position rather than a traditional 9-to-5 schedule. Hours will vary depending on the number of students assigned, school schedules, student availability, service needs, documentation requirements, and coordination activities.
Services will primarily take place during school hours, with additional communication, care coordination, or documentation completed as necessary.
Independent Contractor Status
This position is structured as an independent contractor (1099) opportunity. Contractors are responsible for managing their availability and completing assigned services and required documentation in accordance with contractual requirements and New Vision Health & Wellness policies.
Ideal Candidate
The ideal candidate is someone who genuinely enjoys working with children and adolescents and understands that successful case management involves more than providing referrals. We are looking for someone who will build relationships with students, follow through with families, collaborate with school personnel, identify barriers, and actively help connect students and families with the resources they need.
New Vision Health & Wellness is committed to providing compassionate, culturally responsive, and person-centered behavioral health services to the communities we serve.
