Working within our Health System Improvement division, you will lead partner readiness and onboarding activities, develop referral and care coordination workflows, facilitate training, and support continuous quality improvement initiatives.

You will collaborate closely with project leadership and community partners to translate operational plans into practical, sustainable solutions that improve access to services and strengthen clinical-community partnerships.

What You'll Do

  • Support local health departments in developing and implementing their roles within the Community Care Hub.
  • Provide technical assistance related to staffing, care coordination, referral processes, and operational readiness.
  • Identify implementation challenges and collaborate with partners to develop practical solutions.

Training & Quality Improvement

  • Develop and deliver training, technical assistance, and educational resources to participating organizations.
  • Serve as a subject matter expert in Community Care Hub implementation, care coordination, and clinical-community linkages.

  • Lead partner engagement activities and coordinate implementation plans, training schedules, and key milestones.
  • Facilitate regional and statewide planning meetings, stakeholder discussions, and partner convenings.
  • Support collaborative planning, partner engagement, and Community Care Hub governance activities.
  • Work with project leadership and community partners to support pilot implementation, evaluate progress, and expand the Community Care Hub model into additional rural and tribal communities.

What You'll Bring
