Senior .NET Backend Engineer Consultant (AI-Forward) — Pharmacy Platform

What We're Looking For

We need a Senior .NET Backend Engineer Consultant (AI-Forward) to build the APIs at the center of a pharmacy platform. Prescriptions, orders, claims and patient medication data all pass through the services you'll own. You understand pharmacy workflows well enough to know why a controlled-substance refill, a transfer and a prior-auth denial each need different handling. This is a corp-to-corp, 1099 engagement for an independent consultant who works autonomously and uses AI tools daily to multiply their impact.

Key Requirements

• 10+ Years of C# / .NET
• ASP.NET Core Web APIs on current .NET, EF Core and Dapper.
• Clean, testable service design: idempotency, resilience, observability; unit, integration and end-to-end tests.
• SQL Server
• Strong SQL, including working confidently inside large existing schemas.
• Azure
• Key Vault, Storage, Application Insights, managed identity, containers.
• Securing services with Entra ID; CI/CD with GitLab CI.
• Pharmacy Software (Required, 3+ years)
• Building software for a pharmacy or PBM: prescription lifecycle, NCPDP claims, NDCs and drug data, NPI and DEA, refill and transfer rules, controlled-substance handling.
• Experience with HIPAA-regulated systems and PHI.
• AI-Forward Engineering
• Daily practitioner with AI coding tools and agents: Claude Code, OpenAI Codex CLI, GitHub Copilot or Cursor.
• Prompt and instruction design: translate business rules into structured prompts, tools and context.
• Code review and output evaluation with a human in the loop; you own what ships.
• LLM integration where it earns its place (Azure OpenAI / OpenAI / Anthropic): tool calling, structured outputs, guardrails.
• Safety and compliance: PHI redaction, secrets in Key Vault, nothing sensitive in prompts, logs or test data.

• Confident with engineers, product owners, pharmacists and technicians alike; you turn workflow detail into resilient systems.

• Must have an active LLC or S-Corp; 1099 only (Corp-to-Corp). 2+ years running your own consulting company strongly preferred.

What You'll Do

• Build APIs: Design and build ASP.NET Core Web APIs on current .NET with EF Core, Dapper and SQL Server.
• Own Pharmacy Domain Logic: Prescriptions, orders, claims and medication profiles, including controlled-substance rules.
• Integrate: Connect pharmacy management systems and other healthcare and business partners; work with event streaming and healthcare messaging where needed.
• Secure & Operate: Entra ID authentication, Azure containers, GitLab CI, and the tests that keep releases safe.
• Use AI Well: Apply AI tools to accelerate design, coding, testing and review while keeping PHI inside controlled boundaries.

Preferred Tech & Tooling

• .NET 8+, C#, ASP.NET Core, EF Core, Dapper, SQL Server
• Azure (Key Vault, Storage, Application Insights, Container Apps), Entra ID, GitLab CI
• Kafka or HL7 v2 messaging
• Agentic IDEs: Claude Code, OpenAI Codex CLI, Copilot/Cursor

Nice to Have

• Hands-on with a pharmacy management system's data model or integrations.
• HL7 v2 or Kafka in production.
• Power Platform / Dataverse integration.
• Long-term-care or specialty pharmacy background.
