What they need

About the role

We are looking for a compassionate and motivated Senior Wellbeing Practitioner who is

passionate about improving children and young people’s mental health and helping them

build confidence, connections and opportunities for the future.

Working within Kirklees Young People’s Social Prescribing (KYPSP), you will support

children and young people aged 11–25 who are experiencing emotional wellbeing

difficulties. You will combine structured wellbeing support with a personalised social

prescribing approach, helping young people identify what matters to them and access

activities, relationships and community opportunities that support their wellbeing.

You will recognise that mental health is influenced by a young person’s wider

circumstances, including relationships, education, employment, isolation and access to

meaningful opportunities. Working alongside each young person, you will agree

achievable goals, build on their strengths and help them overcome barriers to taking part

in community life.

Alongside managing your own caseload, you will provide day-to-day guidance and

support to allocated practitioners, model good practice and contribute to the

development of a safe, effective and inclusive service.

You will work closely with the Operations, CAMHS, schools and community partners to

strengthen pathways and improve young people’s experiences of support.

This role requires someone who listens with empathy, values young people’s voices and

brings enthusiasm for both direct work and supporting others to deliver high-quality

care.

Major Duties:

  1. Build trusting relationships and work collaboratively with young people to

understand their strengths, interests, needs and wider circumstances, identifying

what matters to them and agreeing meaningful goals.

  1. Support young people to develop practical skills to manage emotional wellbeing

difficulties, strengthen resilience and increase confidence and independence.

  1. Identify suitable community activities, groups and opportunities that support

wellbeing, belonging and participation, including creative activities, physical activity,

volunteering, education and employment support.

  1. Help young people overcome barriers to accessing support and activities. Where

appropriate, facilitate introductions, support initial engagement and review

whether the connection is meeting their needs.

  1. Develop and maintain relationships with local organisations to strengthen the range

of accessible and inclusive social prescribing opportunities available across Kirklees.

  1. Involve parents, carers and relevant professionals where appropriate, taking

account of the young person’s wishes, consent, confidentiality and safeguarding

needs.

  1. Assess and review risk in accordance with service procedures, seek advice when

required and escalate concerns promptly through agreed safeguarding and clinical

pathways.

  1. Recognise changes in need and the limits of the service’s offer, working with the

wider team and referring professionals to support appropriate onward referral or

additional help.

  1. Manage an allocated caseload effectively, maintaining timely communication,

accurate records, intervention plans, outcome measures and planned reviews and

endings.

  1. Capture young people’s feedback and monitor progress against their goals, using

this information to improve individual support and contribute to service

development.

  1. Work in a way that is inclusive, sensitive to trauma and responsive to individual

communication, cultural, sensory and accessibility needs.

  1. Participate in supervision, reflective practice and continuing professional

development, applying learning to improve the quality of support provided.

Additional Senior Duties:

  1. Provide day-to-day guidance to allocated KYPSP practitioners, supporting consistent

practice in wellbeing interventions, social prescribing, engagement and caseload

management.

  1. Line-manage allocated staff and volunteers, holding regular one-to-one meetings

and supporting induction, development, wellbeing and performance.

  1. Facilitate reflective discussions and shared learning, ensuring that matters requiring

clinical supervision or specialist advice are referred to appropriately qualified

colleagues.

  1. Support the Operations Manager with referral reviews, allocation and oversight of

caseloads, highlighting concerns about risk, waiting times, capacity or suitability for

the service.

  1. Maintain effective working relationships with CAMHS and other partners,

supporting clear communication about the KYPSP offer, referral criteria and agreed

pathways.

  1. Lead community engagement and partnership development, strengthening

opportunities for young people to access meaningful support beyond their

individual intervention.

  1. Identify barriers affecting underserved groups and work with partners and young

people to improve accessibility and inclusion within the agreed referral pathway.

  1. Contribute to service monitoring and improvement by reviewing outcomes,

feedback, engagement and emerging needs, and supporting agreed actions.

  1. Represent KYPSP at relevant meetings and networks, and deputise for the

Operations Manager for agreed responsibilities within the scope of the role.

  1. Undertake designated safeguarding responsibilities following appropriate training

and authorisation, in accordance with organisational procedures.
