Senior/Social Worker

• 5 Day Work Week
• Working location: South Central Singapore, with travel to the West
• Permanent full-time
• Senior positions available depending on experience and qualifications

A leading provider of senior care and community-based healthcare services is currently seeking Social Workers and Senior Social Workers to join their growing team.

Responsibilities:

• Conduct psychosocial assessments and develop person-centred care plans for clients and caregivers
• Provide counselling, emotional support, crisis intervention and practical assistance
• Manage complex cases, monitor clients’ progress and review intervention plans
• Coordinate referrals and services with hospitals, healthcare providers, government agencies and community partners
• Support caregivers and families through counselling, family meetings and education on available resources
• Work closely with multidisciplinary teams to ensure holistic and coordinated care
• Maintain accurate case documentation, reports and service records in accordance with professional and organisational requirements
• Provide guidance and mentoring to junior social workers, interns and support staff where required
• Participate in quality improvement, programme development, service evaluation and organisational projects

Requirements:

• Degree or postgraduate qualification in Social Work recognised by the Singapore Association of Social Workers (SASW)
• Registered Social Worker (RSW) or eligible for registration is preferred
• Min. 2 years of relevant social work experience, candidates with more experience may be considered for Senior positions
• Experience in healthcare, eldercare, community care or case management is advantageous
• Willing to travel to West Region when required
