Position Title: Service Coordinator

Title of Immediate Supervisor: Program Manager

Job Classification: Full-Time

Salary Range: $51,982 - $76,801

Summary Of Position Provides case management services for an assigned caseload. Works closely with individuals we serve and/or their families. Communicates with staff, vendors and the community. Organizes and resolves work-related situations.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Contacts assigned individuals we serve and documents; contacts according to established SCLARC Standard Operating Procedures (SOPs) and policy, and completes required documentation.

Prepares case transfers as needed.

Attends all unit, division, supervision, staff and in-service training meetings as required.

Attends all Planning Team meetings at primary programs and any other individual we serve related special meetings/hearings (e.g. SCAN, court, IEPs/IFSPs staffings, mental health, governmental/community agencies, placement, etc.) as required.

Handles officer-of-the-day (OD) duties as required.

Handles confidential information and adheres to Welfare and Institution (W&I) codes.

Consults with appropriate specialists regarding the service needs of individuals we serve.

Maintains effective working relationships with top management, peers and staff.

May act as Program Manager in his/her absence.

Supervisory Responsibilities This position has no supervisory responsibilities. However, one may be asked to act as Manager in the absence of the Program Manager.

EDUCATION And/or EXPERIENCE Bachelor’s degree (B. A.) from four-year college or university; two years related experience and/or training; or equivalent combination of education and experience. Internships and/or training level employee may have less requirements.

CERTIFICATES, LICENSES, REGISTRATIONS California Driver License, Class C with personal auto insurance (meeting minimum California requirements).
