Job Summary
The Shared Living Service Coordinator plays a vital role in supporting individuals with disabilities by coordinating and overseeing shared living arrangements. This position involves working closely with clients, families, and community partners to ensure a safe, respectful, and empowering living environment. The coordinator is responsible for developing individualized plans with the case manager, providing ongoing support, and ensuring compliance with relevant regulations. Strong communication and writing skills are essential to effectively document services and facilitate collaboration. Experience working with individuals with disabilities and culturally diverse communities is highly valued to promote inclusive and culturally sensitive support.

Duties

• Coordinate shared living placements, ensuring they meet the unique needs of each individual.
• Develop and implement personalized support plans in collaboration with clients, families, and service teams.
• Provide ongoing support and supervision to shared living providers and residents to promote a positive living environment.
• Maintain accurate documentation of services, progress notes, and compliance reports through clear and professional writing.
• Facilitate communication among all stakeholders, including clients, families, community agencies, and service providers.
• Conduct regular assessments to ensure the well-being of residents and address any issues promptly.
• Assist in training and onboarding new shared living providers to ensure understanding of policies and best practices.
• Promote cultural competence by working effectively with diverse communities to respect individual backgrounds and preferences.

Qualifications

• Proven experience working with individuals with disabilities in a supportive or coordinating role.
• Excellent writing skills for documentation, reports, and communication purposes.
• Strong interpersonal skills with the ability to collaborate across multiple stakeholders.
• Knowledge of regulations related to shared living arrangements and disability services is preferred.
• Ability to manage multiple priorities while maintaining attention to detail in documentation and service delivery.
• Compassionate approach with a commitment to empowering individuals toward independence and community integration.

Pay: From $52,000.00 per year
