Skilled Nurse (Licensed Practical Nurse or Registered Nurse) – Home Setting

Duties

• Assess patients' needs and implement appropriate safety measures and interventions
• Carry out the nursing care plan under the supervision of the Director of Nursing
• Administer prescribed medications and treatments, including medication changes per physician orders
• Observe and promptly report changes in a patient's physical or mental status to the nursing supervisor
• Accurately document nursing observations, activities, and treatments
• Communicate effectively with family members, office staff, and supporting health professionals

Requirements Needed:

• Current Pennsylvania Skilled Nursing License (LPN or RN)
• Current CPR certification
• Ability to lift at least 50 pounds
• Ability to pass state and federal criminal background checks. Background checks are individualized assessment tied to the specific job duties of the case.
• Up-to-date physical and two-step PPD or chest X-ray
• Ability to pass a skilled nursing pre-employment test provided by Personal Health Care
• Valid driver's license and a reliable vehicle (SEPTA-only won't reach most of these client's homes)

Job Summary
Personal Health Care, a Medicare-certified home health agency serving Bucks, Chester, Delaware, Montgomery, and Philadelphia Counties, is seeking a caring and reliable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide skilled, one-on-one nursing care to clients in their homes in the Ambler, PA area. Full-time and part-time positions are available, with weekday and weekend shifts, plus additional clients throughout the surrounding area.
