Responsibilities

• Conduct client assessments to identify needs related to mental health, healthcare, and social support systems
• Develop personalized care plans and connect clients with appropriate resources such as Home Health and/or hospice programs and behavioral health services
• Provide crisis intervention and crisis management support during urgent situations to ensure client safety and stability
• Utilize motivational interviewing techniques to encourage positive behavioral change and foster client motivation
• Maintain accurate case management documentation, including progress notes, service plans, and referral records
• Collaborate with community organizations and social service agencies to coordinate comprehensive support for clients
• Offer mentoring and client communication to build trust and promote ongoing engagement in services

Skills

• Strong knowledge of social services systems including Medicaid, Medicare, and behavioral health resources
• Experience working with seniors, individuals with disabilities, or geriatric patients in diverse settings
• Proficiency in crisis intervention strategies and crisis management techniques
• Ability to identify community resources effectively and assist clients in resource navigation
• Excellent writing skills for documentation and reporting purposes
• Bilingual abilities are highly valued to serve culturally diverse populations effectively
• Familiarity with social work principles such as motivational interviewing and case management
• Knowledge of first aid and CPR certifications are preferred for emergency response readiness
• Experience working with culturally diverse communities to promote inclusive service delivery
