Social Services Assistant

Heritage Grove Estates is seeking a Social Services Assistant to support the social, emotional, and psychosocial needs of residents in a long‑term care setting. This position works under the direction of the Administrator and Director of Social Services.

Key Responsibilities

• Implement the facility’s social services program
• Assess resident social service needs and coordinate resources
• Assist residents and families with discharge planning and community referrals
• Coordinate care with the interdisciplinary team (IDT)
• Participate in care planning, MDS social service components, and resident councils
• Provide education on resident rights, advance directives, and available services
• Assist with admissions, transfers, and discharges
• Maintain compliance with state and federal regulations

Qualifications

• Bachelor’s degree in Social Work or related human services field
• Minimum one (1) year supervised social work experience in a healthcare setting. Candidates who have completed their social work internship are encouraged to apply.
• Strong communication and documentation skills
• Ability to work effectively with residents, families, staff, and community agencies

Physical Requirements

• Ability to lift, push, or pull up to 25 lbs
• Ability to move throughout the facility and assist during emergencies as needed
