At Sundance, we believe great care starts with people who genuinely care. Our team supports residents through short-term rehab and long-term care, and we’re looking for a compassionate Social Services Assistant to help residents and families feel heard, supported, and at home.

What You’ll Do

• Assist the Social Services Director with resident admissions, orientation, and adjustment to the facility
• Complete psychosocial assessments and help with care plan updates and care conferences
• Serve as a resident advocate, helping with concerns, grievances, and resident rights
• Coordinate discharge planning, including referrals to home health, equipment, and community resources
• Help residents and families with Medicaid applications, benefits, and other paperwork
• Keep accurate, timely documentation in the medical record
• Work closely with nursing, therapy, and activities to support each resident’s emotional and social well-being

What We’re Looking For

• High school diploma or GED required; bachelor’s degree in social work, psychology, or a related field preferred
• Experience in long-term care, healthcare, or social services preferred
• Strong communication skills and patience with residents, families, and staff
• Organized, detail-oriented, and comfortable with computers and documentation
• A warm, respectful approach to older adults and people with disabilities
