Social Services Case Manager

This role requires critical thinking, effective communication, and decisive judgment to build positive relationships and lead others. The incumbent must be able to take appropriate action when necessary.

Responsibilities include: providing social work services, supporting patient care, and collaborating with healthcare teams to address psychosocial needs.

Qualifications: at least one year of relevant work experience and a licensed Clinical Social Worker license in Florida for specific areas such as Dialysis, Transplant, Cystic Fibrosis, and mental status exams in the Emergency Department. Educational background in Psychology, Mental Health, Marriage and Family, or related social services is preferred.
