Job Summary:

We are seeking a compassionate and dedicated Social Worker to join our team at Venice Health and Rehab, a leading provider of short-term rehabilitation, long-term care, and respite services in Venice, FL. Responsibilities:

• Assess the social and emotional needs of residents and develop individualized care plans to address these needs
• Provide counseling and therapy to residents and their families to address issues related to loss, grief, and adjustment to long-term care
• Facilitate resident interactions and activities, promoting socialization and a sense of community
• Collaborate with the interdisciplinary team to ensure comprehensive care and support
• Maintain accurate and detailed records of resident interactions and care plans
• Develop and implement programs to promote resident engagement and satisfaction
