Summary/Objective:

The Social Worker will provide medically-related social services to attain or maintain the highest practicable physical, mental, and psycho-social wellbeing of each patient.

Qualifications for the Social Services Director/Social Worker:

• Must have prior LTC experience
• Bachelor’s degree in social work or similar field.
• Two years’ experience in a social worker role, preferably in a health care setting.
