Social Services Worker

About the Role

Rebekah Rehab and Extended Care Center is seeking an experienced, compassionate and highly-motivated social worker to lead the Social Services department. In this role, the social services director will be the primary advocate for our residents, ensuring their psychosocial, emotional and mental health needs are met.

Checklist for Applicants :

• Education: Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited program.
• Experience: 3+ years of supervised healthcare or long-term care experience.
• Licensure: Check your specific state's board of social work regulations, as many require a state-issued social work license
• A minimum of a Bachelor’s Degree in Social Work (BSW) or a Bachelor’s degree in a human services field (such as sociology, gerontology, or psychology).
• At least 3 years of supervised social work experience in a healthcare setting working directly with individuals (preferably long-term care or geriatrics).

Core Roles and Responsibilities :

Beyond the paperwork and credentials, a nursing home social worker is expected to manage several critical functions:

• Psychosocial Assessments & Care Planning: Evaluating new residents upon admission, addressing depression or adjustment issues, and collaborating with an interdisciplinary team (nurses, therapists, dietary) to draft comprehensive care plans.
• Discharge & Transition Planning: Helping residents safely return home or transition to alternative lower-level care environments (assisted living, rehabilitation).
• Resident Advocacy: Acting as a liaison for resident rights, ethics, and self-determination under the NASW Code of Ethics.
• Family Support & Counseling: Guiding families through the emotional toll of aging, end-of-life care decisions, and navigating Medicare/Medicaid complexities.
• Salary: Based on education, qualifications and experience salary starts at $65,000.
