Social Work Assistant (2-years Contract)

You will assist the Medical Social Workers in carrying out administrative work, in the delivery of social services to patients and their families. This includes processing of information and statistics on assistance through an electronic system. You may be required to gather information from patients and their families and assist them with the financial assessment procedures. You may also be required to conduct home visits.

Responsibilities

• To ensure accessible health and social care through seamless discharge by providing care planning, coordination and follow up
• To provide quality psychosocial support care
• Timeliness and accuracy of fund applications
• History Taking/Documentation and Psychosocial Assessment

Job Requirements

• Diploma preferably in Social Work / Community Service / Human Services
• If Degree in other disciplines, is or will be pursuing Post –Graduate diploma in Social Work
• Relevant experience in Social Services, Accounting and Statistics will be an added advantage
• Proficient in Microsoft Office applications
• Good interpersonal and communication skills
