Social Work Associate (Homecare)

• 5 days work week / Near Marymount MRT / Transport provided at MRT
• Prior exp with social work in hospital setting is preferred

Our client is a leading Healthcare provider

Responsibilities:

• Within Department:
• Send daily messages to new Homecare patients and their MSPs to inform them to complete the Homecare Patient Distress Screener.
• Collate the responses on the Homecare Patient Distress Screener daily.
• Refer to HC PSS the list of patients who scored ≥5 and to follow-up on those patients with scores of ˂5 on their psychosocial and spiritual needs.
• Follow-up phone calls and/or home visits to patients who scored ˂5.
• Record all assessments and interventions into system.
• Attend meetings if and when needed.
• Provide monthly reports on the scoring of the Homecare Patient Distress Screener.
• Any other duties as and when assigned by the HOD and/or supervisor
• With other departments:
• Liaise with Homecare Admin on the daily list of new Homecare patients/MSPs.
• Discuss psychosocial and spiritual needs of Homecare patients

Requirements:

• Diploma in Social Work, Social Services, community development related disciplines, preferably with 1-2 years of experience in the health and/or social service sector
• At least 1 years of social work experience in a healthcare environment preferably
• Bilingual and ability to communicate in local dialects preferred.

Other Information:

• Working Days/Hours: Mon to Thurs (8am to 530pm), Fri ends at 5pm
• Location: Near Marymount MRT (Transport provided at MRT)
• Up to S$4K Basic+allowances+bonuses!
