Social Work Case Manager

Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have experience in hospital settings and a strong understanding of case management, behavioral therapy. This role involves working closely with individuals and families to provide support, resources, and interventions that promote mental health and well-being. The Social Worker will be responsible for assessing client needs, developing treatment plans, and ensuring compliance with HIPAA regulations.

Responsibilities

• Conduct comprehensive assessments of clients’ social, emotional, and environmental needs.
• Develop and implement individualized treatment plans based on diagnostic evaluations.
• Provide case management services to assist clients in accessing necessary resources.
• Utilize cognitive behavioral therapy techniques to support clients in achieving their goals.
• Maintain accurate records of client interactions and progress in compliance with HIPAA standards.
• Collaborate with healthcare professionals to coordinate care for clients within hospital systems.
• Advocate for clients with autism and other behavioral health challenges to ensure they receive appropriate services.

Requirements

• Bachelor’s or Master’s degree in Social Work from an accredited institution.
• Social Worker wanted for an Adult Day Care . MUST be a Licensed Social worker or Certified Social worker to apply. New Grads with LSW or CSW are welcomed to apply.

Duties include:

• Assists clients with accessing social services, including completing Medicaid and SNAP applications.
• Compiles social history and conducts a psycho-social assessment including formal and informal support systems, mental and emotional status, community and financial resources, and care giver data.
• Provides discharge planning and assists in the transition and follow-up as appropriate.
• Contributes to a positive work environment and overall team effort.
• Performs related duties and responsibilities as required or directed by the administrator.

Job Types: Full-time, Part-time

Pay: $33.00 - $35.00 per hour
